[Collection of forms to be used in church business], 1696 and later.

Manuscript on paper, in two different hands, of form documents, including Episcopal licenses, dispensations, inhibitions, and suspensions. Specific forms include "A Dispensacion for an Incumbent to live out of his house belonging to his Parsonage or vicarage," "Absolucon by the Bishop for Contumacye," "A licence to teach an English schoole," "The forme of an Indorsmt. for a certificate of Recusants," "A Licence to Practise Physicke," and "Before the licence to Marrie be given...these things to be observed." Recipes have been written on blank leaves at the beginning and the end in the latter hand, including a recipe for "Dorkroot Ale," "An Excellent purging Dyet Drink," and various tobaccos and wines.
