Each file contains proof of descent and a questionnaire, that lists the name and address of the applicant; biographical information about the ancestor (birth, death, marriage, county and township settled, date settled) and a lineage chart tracing the descent of the qualifying ancestor to the applicant. Includes an index on microfiche offering name of applicant, qualifying ancestor, and county of ancestor.
