Agent Orange victims initial survey questionnaires, 1980-1982.

The Agent Orange Victims program was established under Sections 221, 20.435(1)(R), Laws of 1979. Survey questionnaires and informational correspondence sent by the Wisconsin Division of Health and Social Services as part of the Agent Orange Identification and Assistance Project aimed at Wisconsin veterans who served in Vietnam between 1962 and 1971. Questionnaire ascertains veterans' foreign military experience, perceived health problems, and Agent Orange exposure. The questionnaire also provides information on obtaining medical evaluation through the Veterans Administration Hospital system.
