Inpatient case files, 1878-1930 (bulk 1878-1929).

Currently under the supervision of the Dept. of Mental Health, Danvers State Hospital and its predecessors have been responsible for providing residential care and treatment of mentally ill persons in northeastern Massachusetts since 1878. Inpatient case files are created to provide a record of the commitment, diagnosis, treatment, and care of patients admitted to the hospital. Files contain some or all of the following: admission record; records of physical and mental examinations; social service and after-care reports; clinical and medical records and charts; chronological notes on patient's condition; autopsy report; correspondence with interested parties; and photograph. Information includes name; registration no.; institutional no.; date of discharge/death; and personal and family history.
