Admission and discharge registers, 1881-1960.

Log noting for each admission: date last admitted, hospital case number, number that year, number male or female that year, name, age, marital status, county of residence, by whom committed, type of admission (voluntary or not), birthplace, occupation, address of guardian, brief diagnosis, number of times in hospital, and religion. For discharges: similar information, plus type of discharge, to whom discharged, and condition when discharged. Statistical summaries are included at end of each month.

The Wisconsin State Hospital for the Insane opened in 1860. It was built on a site near Madison purchased by commissioners of the Wisconsin State Lunatic Asylum (authorized in 1854 but never constructed). In the 1930's the institution began to be known as the Mendota State Hospital and in 1973 it became the Mendota Mental Health Institute.
