Born in 1816 Rev. Lonsdale Formby was an Anglican minister of Formby near Liverpool, England. He graduated from St. Catherine College in Cambridge in 1842. Formby was named Perpetual Curator of Formby, Chester Diocese in 1847. He was also the Patron and Rector of Walton-on-the-Hill. Formby died in 1896.
