All states which are approved for a Medicaid program under Title XIX of the Social Security Act are required to develop and maintain a State Plan for Medical Assistance. Specified in 42 C.F.R., Part 430, Subpart B, the Plan and its accompanying amendments are a comprehensive statement submitted by the State agency describing the nature and scope of its Medicaid program and giving assurance that it will be administered in compliance with specific requirements stipulated in the pertinent title of the Social Security Act and other applicable official issuances of the Department of Health and Human Services (DHHS). The State Plan contains all descriptive narrative necessary for DHHS to determine whether the State's Medicaid program will be eligible for Federal Financial Participation. Pages to the Plan are amended on an as needed basis as well as based on Federal mandates. The files on all Plan Amendments contain copies of the superseded pages, new amended pages, letters to the Health Care Financing Administration (HCFA) regarding the amendment, HCFA Form 179, staff notes, research notes, and general backup material.
