Admission permits, 1854-1929.

The State Almshouse at Tewksbury, established as a residence for paupers of the Commonwealth, evolved after closing of other state almshouses in 1872 into an institution for ill or infirm public charges who could not be transferred to the State Workhouse (later State Farm) or otherwise discharged. Admission was contingent upon receipt of a certifying statement (printed form per St 1856, c 171) from local authorities as to lack of settlement (St 1852, s 275, s 5) or from state welfare authorities (St 1859, c 255; RL 1902, c 85, s 7). Permits document legal admission of persons to the institution. Information for 1920-1929 may include date, registration no., name, date of birth, birthplace, if applicant has legal settlement in a municipality, last address, name and address of nearest relative, disease, reason for sending applicant, remarks, and signature of authority requesting admission. Verso of permit (stamped form) may contain admission date, correct name, birthplace and date, occupation, religion, civil condition, military service, years in U.S. if foreign born, recent injuries, industrial compensation, money, other valuables, diagnosis, who to notify in case of serious illness, special requests, directions for burial, addresses of relatives or friends, admitting doctor, witness, and person bringing applicant.
