Agency forms list city/town name; department/office and subdivision name; establishment date; administrative reorganization description; agency building name and location; other buildings occupied and occupation dates; archives loss or destruction with damage date, cause and extent; "useless" paper policy; sending archives to Washington headquarters; information source; additional information sources; record condition comments; agency handling procedures; project supervisor's name; and inventory date. Serial forms list city/town name; building name and address; room number; custodial and originating agencies or subdivisions; serial title; inclusive dates; serial description noting type and organization; use frequency; index or inventory description; archives physical form (i.e., size and volume (i.e., linear feet)); container number and type; physical damage or deterioration; information sources; fieldworker's and project supervisor's names with the inventory date. Building forms list city/town name; building name and location; construction date and type; fire hazard danger, whether there has been any fire or other damage (i.e., dates; damage extent); precautions and protective devices; and project supervisor's name with inventory date. Room forms list city/town name; building name and address; room number, use, pest and archival impediments; and project supervisor's name with the inventory date. File also includes Historical Records Survey official correspondence concerning survey and administrative histories.

The Illinois Historical Records Survey (1935-1943), created in 1935 as part of the Federal Writers' Project of the Works Progress Administration (WPA) located and described Illinois federal, state, county, municipal, and church archives.
