O'Keeffe was born 1926, in Kinsale, County Cork, Ireland, studied at Oxford, and served in the National Service. In the 1950s, he started his career in publishing at Hutchinson, Ltd., and, in 1959, went to work at MacGibbon and Kee in London. O'Keefe's publishing activity focused on Irish literature. In 1971, O'Keeffe formed the publishing firm of Martin, Brian and O'Keeffe; he continued to run this firm out of his London home until his death in 1994.
