Special Committee on Forms of Service (also known as the Committee on Forms and Services) was appointed by the 1902 General Assembly (PCUSA) to prepare a standardized manual of forms for special worship services. Henry Jackson Van Dyke was appointed Chair. The Committee produced a manual in 1906 entitled: The Book of common worship.
