Cornelius Rea Agnew was born on 8 August 1830 in New York City. He graduated from the College of Physicians and Surgeons of New York in 1852, having as his preceptor John Kearny Rodgers. Agnew was a founder of the Ophthalmic Clinic in the College of Physicians and Surgeons of New York, the Brooklyn Eye and Ear Hospital, and the Manhattan Eye and Ear Hospital of New York.
